Families wishing to apply for the grant must fit in to the following criteria:

Applicant must be aged 5 – 18
Applicant must have a Health Care Card or Pension Concession Card or must be referred by a “recognised referral agent” (such as school teacher)

Applicant must live in the Shire of Wanneroo boundaries. If you live within another Shire, please contact Kathy Monck (0428 113 768) to discuss your application.

Steps to Apply

1. Visit the Shire’s website: www.wanneroo.wa.gov.au/Lifestyle/KidSport
2. Click on Kidsports Application Voucher
3. Fill in details

Post your application to:

Or Hand Deliver to: City Of Wanneroo Civic Centre
23 Dundebar Road
Wanneroo (between the hours of 8.30am – 5.00pm)

4. Once your application has been approved the Shire will give/post you a voucher.
5. Hand this voucher to either:
   • Tim Lines (Treasurer)
   • Kathy Monck (President)
   • Mandy Lines (Registrar)
   Or post to P.O. Box 2128, Clarkson WA 6030
6. The Club will then invoice the Shire on your behalf and provide you with any uniforms you may have ordered. The Shire will grant up to $200 per player for approved applications

7. You can also apply for the cost of the uniform. If you would like to order any uniform please contact Mandy Lines via email ASAP before we invoice the City of Wanneroo

4. If you have already paid for your fees, upon receiving this voucher, the club will reimburse you via cheque.
